CRIMINAL RECORD CHECK INFORMATION

Thank you for applying on-line for a Criminal Record Check.

In order to proceed any further you MUST advise the Administrator, by email, of your Full Legal Name , Hockey ID # if in the HCR and the Comox Valley Minor Hockey Player you are associated with.

Upon completion you will receive an email back from the Administrator with the Access Code to complete the Criminal Record Check on-line and at no cost to yourself. Please follow the link provided and complete process in its entirety.

While you are completing the Criminal Record Check process you will be provided with a Session ID # which the Administrator will need emailed back to change your Hockey Profile to Criminal Record Check completed. Without this Session ID # this part of the process cannot be completed .
